Supporting individual goals

Where support happens
Support may take place at home or in community settings, according to the authorized service and the person’s goals.
What to expect
Staff learn the person’s routines and preferences, provide the level of support authorized, and document the assistance provided and the person’s response.
Working with families and WSCs
Holley Star communicates with the person and appropriate stakeholders to clarify schedules, authorized outcomes, changing circumstances, and next steps. Service documentation supports continuity and accountability while protected information is shared only through approved channels.
